Nothing I've seen in my lifetime can come close to topping the original cover of This Will Destroy You's self-titled album.

Unknown Pleasures absolutely kills the competition at the minimalist end of the spectrum, though.

I can't believe no one has mentioned any Robert Pollard or Guided By Vocies Album Covers. Mr. Pollard makes his own cover art, which is very unique. He has this very pop art/surrealist style to his covers. Check some of these out.
