I don't see it at all, personally. Examples why:
Gorillaz - Demon Days
Muse - Origin of Symmetry
Radiohead - The Bends
The Mars Volta - Frances the Mute
Jefferson Airplane - Surrealistic Pillow
All of these I found to be much stronger than their debuts.

So what I've learned from this discussion is that the sophomore slump isn't a huge problem for bands / artists with staying power, that is, the kinds of bands / artists we celebrate on this site. But the MTV-type artists who are signed to cash in on the latest trend often fall victim to this and quickly become has beens as they weren't cut out for greatness to begin with. Now great bands / artists do fall prey to the sophomore slump, but the drop in quality usually isn't as huge as it is with the flavor of the month bands who will soon be forgotten or in the category of "whatever happened to...?"

I have to make another correction. Zen Arcade was Husker Du's third album after Everything Falls Apart, which I think was out of print for a while. Space Oddity was David Bowie's third album after my favorite, Hunky Dory, and also I argue against your perceived greatness of Space Oddity as an album. Lust for Life is actually Iggy's debut as a solo artist. His second album is The Idiot.

The Space Oddity error may be due to the circumstances around it's release. It was first released under two other names in 1969, David Bowie in the UK, Man of Words/Man of Music in the US. In both cases it was Bowie's second effort. Then came The Man Who Sold the World in 1970 and Hunky Dory in 1971, before it was actually releases AS Space Oddity in 1972.
So Space Oddity, it could be argued, is Bowies FOURTH album. But of course it should be argued that it's his second.
