...and they started smoking. Like, would you be cool with that? It is their body and they can do what they like with it... except it is your body as well. You wouldn't be smoking but at the same time you'll get most/all of the health problems from it. It's a bit of a jerkish thing for them to do. I mean, would you want your conjoined twin to start smoking? :?

Oh, to actually answer the topic question:

It all depends on what organs you share. However, even if you had separate lungs, your twin's cancer would eventually enter your half of the body.
