Since Iron Maiden's Dance Of Death has already been mentioned, I'll go with this incredibly unoriginal atrocity (a description that could fit both the cover and music)...

Drowned In A Sea Of Sound by The Daysleepers

Only because the music is so freaking good, but this cover just seems so lazy and does not reflect how good the album is. It looks like a Sleeping With Sirens album cover.
