Doesn't have much 'wow' factor, but this album has slowly grown on me and is now one of my favourites. Perfect album to listen to when tired/moody. Melodies are really good.

Does anyone know what 'everybody's dying just to get the disease' is referring to?

I like Elliott Smith's stuff, but it seems like his catalog is played to death in the circles I run in. This is very good, and I can't say I notice a huge difference in quality from one of his albums to the next.
