I'm not sure if Facebook has got this right or not (and worth borrowing from) as appreciate it's a very sensitive topic, but the way Facebook do things could be a way of doing it here i.e Male/Female/[Other] where other can be chosen from a list. This would be added to the new version of the forums which are being worked on in the background (and aren't too far away).
