The idea that you have people who are 'uninsurable' because of their medical history and therefore won't be treated should they fall ill is surely pretty repulsive.

As for what constitutes a 'fair' amount of tax, well that varies from person to person but no developed country has a flat rate of tax, the implication being that these countries feel it is fair to tax people with higher incomes at a higher rate. See people with low incomes already have services provided for them by the state in excess of the tax they pay to fund these services. The policies of both parties in the US would not end this, both parties seem content with the fact that poor people claim more from the government than they pay in taxes.

The healthcare bill I suppose would increase the gap between what poorer people pay in taxes and the services they receive for this, but whether that's 'fair' or not is entirely subjective. My viewpoint is that healthcare shouldn't be treated as a commodity but more like a right, and one that any developed country should be able to afford it's citizens. There's plenty of ways a country can do this but it certainly does look weird when a country as rich as America decides not to.

Would it be fair to require people to maintain a proper BMI; to get the proper nutrition, hydration, excercise, rest and education, and; to not smoke in order to get tax favored treatment/subsidies?

In Canada, I don't have to do jack. My health care is covered.
